COMPATIBILITY
- The fillable binder is a PDF with standard form fields: fill it in on a computer in a PDF reader that supports form fields, and save often
- Typing, ticking and saving were tested with the PDF engine that Chrome's built-in viewer uses. Adobe Acrobat, Microsoft Edge and Apple Preview were not tested
- Or print it blank and write by hand
GOOD FOR
Parents, grandparents, foster parents and other caregivers of a child with an IEP or a 504 plan, including children with autism, ADHD, speech delays or learning differences. Bring it to IEP meetings, parent-teacher conferences and therapy appointments.
PLEASE NOTE
- US Letter size (8.5 x 11 in). On A4 paper, choose Fit to page when printing
- Written for US IEPs and 504 plans in general; it does not follow any one state's or district's forms
- One binder per child: keep a blank copy and save a separate copy for each child
- By design there are no fields for full student ID, insurance or account numbers: write the last 4 digits only
- Typed text in the larger notes boxes does not follow the printed ruled lines. For neat handwriting, print the page
- The child shown in the photos (Sam) is a made-up sample. Your files arrive blank
- Once filled in, this binder holds private information about your child: keep the printed copy and the file somewhere secure
- For your own household's use; please don't resell or share the files
